Biliary dyskinesia: when pain without stones needs further investigation

Ультразвуковий датчик у кабінеті діагностики

Biliary dyskinesia is a term often used for problems with the functioning of the biliary system without obvious stones. However, pain under the right ribs and a “clear” ultrasound do not establish this diagnosis. The nature of the pain, blood test results and other possible explanations for the symptoms need to be assessed first.

This article concerns adults and primarily functional gallbladder disorder. Approaches for children and other biliary disorders may differ. Its purpose is to help you prepare for a consultation, not to choose treatment from a description of symptoms.

What matters more than the word “dyskinesia”

The doctor needs to understand whether the pain resembles biliary pain. These are episodes of significant upper abdominal or right upper quadrant pain that interfere with usual activities. Duration, recurrence and the relationship to bowel movements, body position or medicines are important.

The Rome Foundation criteria for functional gallbladder disorder are based on typical biliary pain and the absence of stones or other structural disease. The diagnosis should therefore not be made from one vague symptom, such as a bitter taste in the mouth, or an incidental description of the gallbladder’s shape.

If the ultrasound report mentions a fold, ask what it means in your situation. The separate page on a gallbladder fold can help explain the terminology. A word in a report does not replace a clinical assessment.

How to describe your pain

It is useful to keep a short diary of episodes before your appointment. Do not deliberately trigger pain with food or stop prescribed medicines. Record what happens in everyday life and bring your notes together with test results.

  • Exactly where the pain occurs and whether it spreads to the back or shoulder.
  • How long an episode lasts and whether there are completely pain-free intervals.
  • Whether you have to stop work, wake up or seek help.
  • Whether there is a relationship to food, bowel movements or changes in position.
  • Which remedies you have tried, in what order and with what result.

A precise description such as “it hurt for forty minutes after dinner and I had to lie down” is usually more informative than “my gallbladder does not work well”. It is also important to explain what troubles you most: attacks of pain, persistent bloating, nausea or fear of eating. These may require different lines of investigation.

What ejection fraction means

In some situations, a specialist may discuss testing gallbladder contraction, including hepatobiliary scintigraphy. Ejection fraction describes its emptying under the conditions of a particular test. It is not an overall score for the health of your digestion.

The SAGES guideline on biliary surgery considers abnormal emptying together with symptoms. At the same time, a scientific review of the diagnosis and treatment of biliary dyskinesia highlights the limited high-quality evidence concerning surgery.

A low ejection fraction alone does not mean that the gallbladder must be removed. Ask why the test has been recommended, how its result will change the plan and how confidently the symptoms can be attributed to the gallbladder. Do not compare numbers from different examinations without a specialist’s explanation.

How to discuss treatment

The plan should take account of symptom severity, the investigations already performed and the expected benefit. If surgery is suggested, ask exactly which symptom it is intended to improve, what alternatives exist and what will be done if pain persists.

The general stages of the operation are described on the Gallbladder removal page. However, learning about the technique does not answer the main question: are there sufficient indications in your case? This decision requires individual discussion without promising that all digestive complaints will disappear.

When not to blame everything on dyskinesia

Fever, jaundice, worsening or prolonged severe pain, and repeated vomiting need urgent assessment. If fainting, confusion or sudden deterioration occurs, call 103. A previously diagnosed functional disorder does not automatically explain a new acute illness.

Frequently asked questions

Does a normal ultrasound mean there is no pain?

No. It only answers certain diagnostic questions. Real symptoms deserve assessment even when no stones are found.

Can I start bile-stimulating medicines myself?

Do not choose medicines based on a diagnosis name found online. First clarify the cause of the symptoms, the aim of treatment and how its effectiveness will be assessed with your doctor.

Would a second opinion help before surgery?

It may be useful if it is unclear what supports the indications. Bring full reports to the consultation, not just the final number from one test.
